Physician Availability

  • Medical Director rounds weekly. The MD rounds to all residents needing care each week including all new admissions within 5 days of admit. The Medical Director reviews all nurse notes and meets with the Nurse Practitioner for resident updates and care planning.

  • Cardio provider rounds twice weekly. Our cardio provider partnership supports heart and lung health by providing regular monitoring, personalized treatment, and expert oversight tailored to each resident’s cardio care needs.

  • Psychiatry provider rounds weekly. Our psychiatric care partnership supports residents’ mental health by providing regular assessments, personalized treatment plans, and expert guidance to promote overall emotional well-being.

  • Wound Care Physician rounds weekly. Our Wound Care physician partnership enhances our wound care program by providing regular treatment and oversight for resident wound needs.

  • Psychologist rounds twice weekly. Our therapy partnership provides regular sessions to support residents’ emotional well-being, offering personalized guidance, active listening, and strategies to enhance mental and emotional health.

  • Certified dietician on staff that provides expert nutritional support by assessing each resident’s unique needs, developing personalized nutrition plans, and managing therapeutic diets tailored to support specific health conditions and care goals.

Specialized Practitioners

The following specialized practitioners visit our facility:

  • Ophthalmologist visits quarterly.

  • Dentist visits quarterly.

  • Audiology visits quarterly.

  • Podiatry visits quarterly.

These specialists come to Chattanooga Health and Rehab Center and provide direct care in our residents’ home ensuring a comfortable location and ease of access to care.

Chattanooga Health and Rehab Center also offers Hospice Services with the provider of choice for residents. Our Hospice teams provide patient-centered care with a variety of service options, including music therapy, massage therapy, chaplain services, end-stage dementia support, and sensory comfort items such as weighted teddy bears or stress balls. In addition, our hospice partners provide palliative and respite care to support residents’ comfort, quality of life, and the needs of their families.
